NW London Diabetes transformation Team wins Quality in Care award

The North West London Diabetes Transformation Team has won the “Quality in Care” award for “Sustaining Quality Improvement and Patient Outcomes’ - In the “Best Practice in Dissemination and Sustainability Award” category at the QIC Diabetes Awards 2019.
The team have been awarded for their work to improve care and provide better outcomes for people living with diabetes in North West London. They have done this through a comprehensive approach to improving the whole patient journey, including prevention of Type 2 Diabetes, an increase in support for effective self-care and improvements in primary care services, and a significant increase in education for hospital clinicians. The result has been an improvement in a range of key diabetes outcomes over the past 4 years, including reducing heart disease risk and foot ulceration rates.
The judges said: "The North West London Health and Care Partnership’s entry is very impressive and it was incredible how much work they have done to collaborate and get this project progressed. They recognised the challenges involved and were clear about what they wanted to achieve. They’ve grown the work and kept it going. It is sustainable and transferable and truly remarkable. "
Dr Tony Willis, Clinical Director for Diabetes, North West London Collaboration of Clinical Commissioning Groups, said “It is great that the team’s hard work and dedication has been recognised. This is a great achievement not only for the team but also the wider community as it ultimately improves the patient experience.”
The Know Diabetes website has been launched (www.knowdiabetes.org.uk) and is a dedicated resource to providing people living with diabetes with support and advice on how to manage their conditions. This is done through a range of e-Learning resources, tailored information and videos.
